4 Important Structures
There is a general structure of a photo consent form that must be followed. Below are steps that can be followed to help you write a PCF the right way
1. Use Adobe PDF, Microsoft Word (.docx), or Open Document Text (.odt).

2. In this step, the release must also clearly expose the revocation clause.

3. In this final step, both sides, both releases or releasors, must sign the PCF at the bottom of the form.

Parental Photo Consent Form
