It turns out that even an esteemed institution is not perfect. Just because they have flaws, doesn’t mean that they have to stick with it. As a patient, you need to help the hospitals see the flaws that they might not realize in the first place. The way to do that is by using a patient complaint form. Read the information about the structure and tips in this article.
The Structure of a Patient Complaint Form
Not all forms are made the same. There are some differences in the form for the patients. Those differences can be seen in the structure:
- Patient’s identity
On the top part of the form, the patient needs to write down some information about them. This includes their full name, date of birth, social security number (optional), address, and contact number. In some cases, they will also be asked about the details of their treatment in the hospital; what they are being treated for, where, and when. - Detail of incident
Then, the patient complaint form will ask patients to give some elaboration about the incidents. This includes the time and place of the incident, followed up by a short elaboration. Patients need to keep it concise. Around four to five sentences will be enough. - Detail of report
The patients also need to fill in some details about the report. When it was reported, who it was reported to are the information asked through the form. - Signature
To authorize the form, both the patient and the receiver will need to sign the form.

How To Give Good Feedbacks
Just because you could give a feedback, doesn’t mean that your feedback will be accepted right away. What you need to do is to craft your feedback so it is better accepted. The way to do that is by following our tips:
- Make it concise
A feedback that is way too long will put off the receiver when they are trying to read it. As we have said, the 4-5 sentence limit is the perfect amount for a feedback. It gives you enough room to elaborate while still capturing the attention of receiver. - Make it objective
Finally, try to make the feedback as objective as possible. Before you write the feedback, ask yourself whether you are giving a substantive or an emotional feedback. - Make it professional
Don’t use cuss words and stick to the formal words. Your feedback will be more acceptable.
How To Hold Them Accountable
Don’t let the patient complaint form sit idle. You need to make sure that your visions are accepted or at least taken into consideration. That is why you need to follow these tips.
The first tip is to know the line of power or authority in the hospital. You need to know who really have the power to make decisions and act on the feedback. Then, you need to correspond to this person or meet them to ask about the progress of the growth. Are they taking the necessary action to reform the hospital? If not, ask them why.
If you are still ignored at this point, that means you need to do something big. You need to gather people who feel the same as you do. Then, you need to ask everyone to back you up to ask for the accountability from the hospital. Make sure that the attempt is legal.
The hospital is flawed, but they can be reformed. The first step for that is through the patient complaint form. Use our structure and follow up tips to get the result that you deserve.
